چکیده

Content/Summary Low pulsatile or non-pulsatile cerebral perfusion was noted in patients with aortic arch dissection, acute proximal internal carotid or common carotid artery occlusion and means impending large cerebral infarction. A 45-year-old man had a sudden aphasia and left hemispheric symptoms with NIHSS of 16 on arrival. Complete occlusion of left middle artery with leptomeningeal collateral circulation from left anterior and posterior cerebral artery was observed on TFCA. After chemical and mechanical thrombolysis, occlusion of left MCA was partially recanalized. TCD showed a non-pulsatile waveform in left middle cerebral artery. In this case, nonpulsatile cerebral perfusion results from MCA lesion, which is more distal rather than proximal occlusion of carotid artery.

Prevention and Management of Cerebral Small Vessel Disease

Lacunar infarcts/lacunes, white matter hyperintensities (WMH), and cerebral microbleeds (CMBs) are considered various manifestations of cerebral small vessel disease (SVD). Since the exact mechanisms of these manifestations differ, their associated risk factors differ. High blood pressure is the most consistent risk factor for all of these manifestations. However, a "J curve" phenomenon in term...

Combined Therapeutic Effect of Probucol and Cilostazol on Endothelial Function in Patients with Silent Cerebral Lacunar Infarcts and Hypercholesterolemia: A Preliminary Study

OBJECTIVE This study evaluated the efficacy of combined therapy with probucol and cilostazol on endothelial function in silent lacunar cerebral infarcts (SLCI) and mild hypercholesterolemia. SUBJECTS AND METHODS Flow-mediated vasodilatation (FMD) and nitroglycerin-induced vasodilatation (NMD) were measured before and after 4 weeks of combined therapy with probucol (500 mg/day) and cilostazol ...

Early detection of microstructural white matter changes associated with arterial pulsatility

Increased cerebral blood flow pulsatility is common in vascular dementia and is associated with macrostructural damage to cerebral white matter or leukoaraiosis (LA). In this study, we examine whether cerebral blood flow pulsatility is associated with macrostructural and microstructural changes in cerebral white matter in older adults with no or mild LA and no evidence of dementia. Diffusion Te...

Lacunar Infarction in Type 2 Diabetes Is Associated with an Elevated Intracranial Arterial Pulsatility Index

PURPOSE The arterial pulsatility index (PI) is measured by transcranial Doppler ultrasonography (TCD) and is postulated to reflect the vascular resistance distal to the artery being examined. An increased PI of the intracranial artery is often reported with diabetes mellitus (DM), old age, hypertension, intracranial hypertension, vascular dementia, and small artery disease. Microvascular compli...

Cerebral Small Vessel Disease and Arterial Stiffness: Tsunami Effect in the Brain?

BACKGROUND Cerebral small vessel diseases, including silent lacunar infarcts, white matter hyperintensities, and microbleeds, pose a risk for cerebrovascular disease, cognitive impairment, and the geriatric syndrome via effects on arterial stiffness. However, the vascular, physiological, and metabolic roles of arterial stiffness in cerebral small vessel diseases remain unclear. SUMMARY Arteri...
